Many people are discharged from hospital too early and return home without appropriate care arrangements in place. For decades convalescent homes provided the ‘in between' care enabling patients to go home with the independence needed to resume their lives. Sadly, convalescent homes have now almost disappeared and the consequence is the increasing trend for hospital re-admissions. National statistics show that, especially where older patients are concerned, the re-admission figures are high with many having to return to hospital within a month.
The People Care Team can provide convalescent care at home via our Home from Hospital Care Service. Upon discharge we can provide a Live-in Carer service which will support a person's recovery and rehabilitation to a level of independence, gradually moving to a number of daily visits.
Clients in Medway may qualify for NHS Homefirst support following discharge from hospital. The People Care Team is a registered supplier of this service to Medway Community Healthcare Trust (MCH). More information about funding is available on our funding page.
More Time for Care - Less Time for Admin
All our carers use state of the art PASS systems to access and update client notes. Using an APP on their smartphone or tablet, carers sign in to a care call electronically, and download the client’s care plan and checklist of tasks for that visit. Each item on the checklist must be completed before the carer signs out so nothing gets missed.
Paperwork is minimised, so carers spend their time caring for their client, rather than writing notes and filling in forms. PASS notes are accessible to clients and their families online, via secure password, giving you reassurance and the peace of mind of always knowing what’s happening, and confirmation that everything’s been done that should have been done.
